documentation. It is not a CMS-defined acronym, universal Medicaid form, or
proof that a service is billable. Map it to every current state, payer, benefit,
provider, service-date, supervision, privacy, and documentation rule before use.

action: coaching, skill practice, navigation, advocacy, connection, planning,
reflection, or purposeful lived-experience-informed support when appropriate.

## P: Participant voice and progress

What did the participant say, choose, practice, complete, decline, or identify
as a strength, progress, or barrier?

## E: Expected next step

What did the participant and peer agree will happen next, who owns each action,
and when will follow-up occur?
