planning, or purposeful lived-experience-informed support when appropriate.

## Participant voice and response

What did the participant say, choose, practice, complete, decline, or identify
as helpful? Clearly distinguish participant-reported information from direct
observation.

## Progress, strengths, or barriers

How did the encounter relate to the participant's goal? Record specific,
role-appropriate progress, strengths, demonstrated actions, or unresolved
barriers. Do not add a diagnosis or clinical conclusion outside the writer's
role.

What happens next, who owns each action, and when will follow-up occur?
